- Photocopy of Form B / Birth Certificate issued by NADRA
- Two recent passport-size photographs
- Photocopy of CNIC of Father or Guardian (if father is deceased)
- Original school leaving certificate duly signed by the relevant board/DEO
- Copy of DMCs for Class 9th to 2nd Year
- Original migration certificate for students from other boards
- Equivalence certificate for O Level students from IBCC, Islamabad
- Affidavit on stamp paper as per institution’s specimen
- Copy of father’s death certificate for orphan candidates
