- Three copies of the intermediate certificate (unattested)
- Four passport-size photographs (unattested)
- Two copies of national identity cards of the candidate and father (unattested)
- Original migration certificate if graduated from a university other than Punjab University
- All submitted photographs and certificates are non-returnable even if admission is refused
